After each session I will compile therapy notes that will be secured in an identified and secured location. Everything that we discuss in the treatment session will be held in strict confidence. I am not allowed to share information about your treatment without your written, prior consent, unless I am compelled to do so by a court order. I am required both ethically and by law to report any instances of suspected child/elder abuse. I am required to contact the person you identify as your Emergency contact if I believe that you are in imminent danger of harming yourself or someone else.

If you request that I provide an invoice for your insurance company, they may ask that I provide them with a diagnosis and a minimal amount of information regarding your treatment. We would complete a Release of Confidential Information if this occurs. If it is required that therapy notes, treatment plans or other written communication be provided, there may be an additional charge.
